This is a general statement of how IGO Dental & Implant Center handles personal and health information collected through this website and our practice. A printed copy of our Notice of Privacy Practices is available at the office on request.
Who we are
This Privacy Policy applies to IGO Dental & Implant Center (“we,” “us,” or “the practice”), located at 512 E Southern Ave, Suite 101, Tempe, AZ 85282, and to igodental.com and any online booking tools we operate. For questions about this policy, call us at (602) 999-8888.
What information we collect
We collect the following kinds of information:
- Contact and identity information: name, date of birth, phone number, email address, and home or mailing address.
- Insurance information: carrier, plan, member ID, and group number.
- Booking and visit information: the appointment time you request, the reason for your visit, the provider you prefer, and any notes you choose to share when you submit a booking request.
- Protected health information (PHI): medical and dental history, examination findings, treatment plans, X-rays, clinical photographs, and other records created in connection with your care. This information is governed by our HIPAA Authorization.
- Communication records: calls, text messages, and emails between you and our team, in both directions. Inbound calls, website chats, and SMS may be answered by our AI receptionist, Lucy AI. Outbound calls and messages from us (appointment reminders, confirmations, recall and reactivation outreach, no-show follow-ups, post-treatment check-ins, and ad-funnel lead callbacks) may also be placed by Lucy on our behalf. Conversations are recorded and transcribed and retained for quality, training, and continuity of care.
- Website usage data: IP address, browser type, device type, pages visited, referral source, and similar information collected automatically through cookies and analytics tools.
How we collect it
- Directly from you when you fill out forms on the website, submit a booking request, call the office, or visit us in person.
- Automatically from your device when you browse the site (cookies, analytics, server logs).
- From your insurance carrier or another healthcare provider involved in your care, with your authorization.
How we use it
- To provide and coordinate your dental care.
- To contact you about appointment requests, reminders, follow-ups, treatment plans, and billing.
- To verify your insurance benefits and process claims.
- To improve our website, services, and patient experience.
- To comply with our legal, regulatory, and professional obligations.
- With your separate opt-in, to send you occasional educational or promotional emails about our services. You can unsubscribe from these emails at any time.
How we share it
We do not sell your personal or health information. We may share it with:
- Other healthcare providers involved in your treatment, such as specialists, primary care physicians, or laboratories.
- Your insurance carrier, to verify coverage and process claims.
- Service providers we use to run the practice. These include our practice management system, our payment processor, our secure email and SMS providers, our IT support, and Lucy AI (the AI receptionist platform operated by Prometheus AI Labs) which handles inbound calls, chats, and outbound follow-ups on our behalf. Each vendor is contractually required to protect your information and use it only for the purposes we authorize.
- Government agencies, courts, or regulators, when required by law, court order, or to protect the safety of patients or staff.
- With your written authorization, to any other person or entity you direct us to share with.
Our AI receptionist (Lucy AI)
To make sure every patient is reached around the clock, we use an AI receptionist named Lucy AI, operated by Prometheus AI Labs, which acts as a business associate of the practice under HIPAA.
Inbound: Lucy answers phone calls, website chats, and SMS messages on our behalf, helps you book or reschedule appointments, answers common questions about hours, insurance, and services, and hands the conversation off to our front-desk team with full context when a human is needed.
Outbound: Lucy also places calls and messages from us on the practice’s behalf, including appointment reminders and confirmations, recall and reactivation outreach for patients who are due for a visit, no-show follow-ups, post-treatment check-ins, and lead callbacks for patients who inquired through a Meta or Google ad.
Voice calls in either direction are recorded and transcribed; web and SMS conversations are stored as text. We use this information to coordinate your care, to keep accurate records of what was discussed, to improve the quality of our service, and to train the system. You can always ask to speak with a human team member, you can opt out of outbound marketing or recall outreach at any time, and you can request that we not retain a particular recording by contacting the office.
Cookies and analytics
We use cookies and similar technologies (such as Google Analytics and the Meta pixel) to understand how visitors use the site, measure the performance of our online advertising, and improve the patient experience. You can disable cookies in your browser settings; some site features may not work as expected if you do.
Text messages and calls
When you provide a mobile phone number, you consent to receive calls and text messages from us about your inquiry, appointment, or treatment. Standard message and data rates may apply. You can opt out of text messages at any time by replying STOP, and unsubscribe from emails by clicking the link in any email we send you.
How long we keep your information
We keep your patient records for as long as required by Arizona law and applicable professional standards. We keep website analytics data for as long as needed to evaluate site performance.
How we protect your information
We use administrative, technical, and physical safeguards to protect your information against unauthorized access, alteration, or disclosure. Examples include encrypted transmission for online forms, access controls on our practice management system, and signed business associate agreements with vendors who handle PHI.
Your choices and rights
- You can request access to, or correction of, the personal information we hold about you.
- You can revoke your HIPAA authorization at any time, as described in our HIPAA Authorization.
- You can opt out of marketing communications at any time.
- You can file a complaint with us or, for PHI, with the U.S. Department of Health & Human Services Office for Civil Rights if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.
Children’s privacy
We provide dental care to patients of all ages, including children. When we collect personal information about a child, we do so with the consent of a parent or legal guardian. We do not knowingly collect information from children online without that consent.
Third-party links
This website may include links to third-party sites (for example, online payment portals or social media profiles). We are not responsible for the privacy practices of those sites and encourage you to review their policies before sharing personal information.
Changes to this policy
We may update this Privacy Policy from time to time. Material changes will be posted on this page with a revised “last updated” date. Your continued use of the site after a change indicates your acceptance of the revised policy.
